Well, hey guys! Welcome back to the Friday Q & A I've been getting a fair number of questions from you all about hair care and hair loss. So for today's Q & A I'm gon na address all of your questions on hair loss and hair shedding and if you're new here welcome, my name is Andrea. I'M a dermatologist and I filmed an the life of the dermatologist vlogs as well as skin care. Q & A focused on a skin care topic of interest. So if this sounds of interest and intrigue to you, I encourage you to stick around and let's get into it. So if you've been noticing like more hair on your pillow in the morning or an increased amount of hair and your brush or in your shower drain, you may wonder: is this hair loss? Why am I I suddenly shedding more hair? So I think one of the most frequently asked questions is what what is the difference between just normal hair shedding and like ex will hear a loss, so it's actually quite normal to shed up to anywhere from 100 to 150 hairs a day. Our hair cycle is in three phases, unlike cats and dogs where, and they shed all their fur at once. For example, a proportion of human hair is always undergoing some shedding, and so those are referred to as telogen hairs and so they're they're supposed to fall out and new ones will should grow in their place. Excessive shedding of telogen hairs, however, is referred to as telogen effluvium, and that can occur in a variety of settings. It'S particularly common in individuals who have experienced a stressful life or medical event, and it typically occurs three months after the event. Such events include things like extreme weight, loss, giving birth simply being under a lot of stress like having to take care of a sick loved one starting a new job, a big move going through a divorce or losing your job if you've been sick, and particularly, if You'Ve had a high fever or, if you've undergone any sort of operation, or this also can occur in the setting of starting and or stopping a new birth control. Pill crash dieting is associated with those people going on juice cleanses, for example. Approximately three months afterwards can have excessive hair shedding and hair thinning and people who exercise - and you know, run marathons, for example, will often experience the telogen effluvium approximately three months after the big race. So another common question that I get is well. When can I expect the success is shedding to stop well, the shedding, as I mentioned, usually starts around three months after the event. If the event was chronic, it may persist a little bit longer. However, in general, the shed the amount of shedding usually Peaks about four months after it starts and then then, as your body readjust, the excessive shedding wall will slow down and then stop usually within six to nine months. The hair shedding will stop and your your hair fullness will go back to what it normally was. However, if you stand or a lot of stress - or there are other stressors that occur in your life, the shedding can be long-lived. People who are constantly under a lot of stress can have long term excessive hair shedding. So what should you do about excessive hair shedding? Well, it's very very important to see a healthcare provider for evaluation and management to make sure there's not an underlying medical cause or vitamin deficiency driving the problem and to better identify what could be causing it. Now. I'Ve talked a fair amount so far about hair shedding or telogen effluvium, but this differs from actual hair loss. Hair loss occurs when something actually stopped. The hair from growing in the medical term for this is antigen. Effluvium antigen refers to the phase of our hair cycle. In which the hair is actively growing, the most common causes of this with hair loss include certain genetic diseases, certain diseases of the immune system, some drugs or medications, hairstyles that pull on the hair or harsh hair care products like chemical, straighteners, relaxers and then some compulsion To pull at one's hair and if you have hair loss, the condition will not stop until these insults are removed so like like excessive hair shedding. However, in order to figure out the cause, it's best to see a healthcare provider for evaluation and management to derm to determine if there is an underlying medical problem and how best to manage it. But those are some common causes. If you suspect that a medicine that you're on is contributing to your hair loss, I also encourage you to talk to your doctor, rather than just stopping in, because some some medications can have serious effects. If you just suddenly an arbitrarily decide to stop them on your own and importantly, other causes of hair loss may require treatment. Many people who have genetic causes for hair loss will continue to lose hair without any treatment and in women who have a genetic background that predisposes them for hereditary hair loss. They often notice a gradual thinning and likewise men who have a genetic tendency towards hair loss. Frequently notice a receding, hairline or sometimes a bald patch that that forms at the center of the scalp, and in these cases there are certain treatments that may help. So, if that sounds like you again, it's best to see a healthcare provider for evaluation and management. Some people have a combination of both things going on and the sooner treatment and oftentimes the sooner. The problem is evaluated, potential offending agents are removed and treatment. And/Or treatment has begun the better. The outcome is for your hair loss. So I get many questions about hair care and and tips for hair care, so how you style your hair can country hair loss and hair breakage depending on what you're doing to it. It can cause it to look frizzy dull lifeless, so here are some tips, avoid using a blow dryer or a flatiron tool on the hair. The high heat dries out. The cuticle coating, the waxy cuticle that coats the hair shaft. It dries it out and makes the hair brittle and susceptible to breakage, and if the hair breaks close to the top of your head man, I actually perceive that as hair loss. So that would be one thing to stop doing for sure and instead air dry, the hair or gently wrap the hair in a non terry cloth, gentle towel or like I do a t-shirt. So it's not ruffle the hair cuticle to be very gentle on the wet strands, keep handling of your wet hair to a minimum, never comb or brush wet hair. It'S always tempting to comb out the tangles, but they'll sort of weave themselves out as the hair dries. There'S actually no need to comb comb them out, just gently towel dry, the hair and as it dries, you know you may find just by running your fingers gently through your hair, that the tangles are no longer an issue. White boys, keeping brushing to a minimum, will also cut down on frizz and fracture of the hair shaft. There'S this old wives tale that you should brush your hair a hundred times a day that actually leads to split ends and brittle hair. So I don't follow that to tip certain products that are marketed as long lasting cold. Those should be avoided and particularly, if you're, using it comb to comb these products through and and in doing it can cause the hair to break and in doing so, lead to hair loss over time. And if you do want to use a flatiron, it should be used. I never to wet hair only on dry hair and on a low setting. Ideally so does not fry your poor hair shaft and try and limit it to no more than the other day hairstyles that are hard on the hair. Shaft and the growing hair include tight, braids weaves cornrows, really really tight, ponytails and hair extensions. This chronic pulling on the hair actually can cause the hair to you, know break, but the growing hair itself can be under so much duress from that chronic traction. It can actually scar down and lead to hair loss and in areas subject to the chronic traction so loose. Hairstyles are really best. Many people ask me about biotin supplements for hair, and I personally take a biotin supplement and have noticed an improvement in the thickness of my hair. Talk to your doctor. If this is right for you personally, I take a bison supplement every day and after approximately three months of continued use, I did notice an improvement in the thickness of my hair, but in addition to that, I also had stopped heat styling. My hair and I'd stop. Blow-Drying my hair and introduced my t-shirt toweling method to the to the line up, so those are other factors that may have contributed to to the improved thickness and density of my hair with the biotin, but I still continue to tehnique it. Biotin is a water-soluble vitamin and it seems to be fairly well tolerated. Some people anecdotally report that it worsens their acne, but that's not something that's been reported in the medical literature. One problem, however, with vitamin supplements is that they're not regulated, so there's no way to know that the supplement that you're taking is any good. So you know if some, if your healthcare provider is recommended, that you take biotin supplements, ask them which one that they recommend for you, and that would be the one to go. But it's largely anecdotal because there's these things aren't regulated. So we have no way of knowing that they actually contain biotin or even what a good amount of biotin to be taking is for this. That'S not agreed upon so, but anyways guys. I hope you found this video helpful today. Judy Luce: As a hairdresser I see a lot of TE with my clients. I have had 3 bouts of TE myself over 10 years. Stress for me brings it on just like you suggested several months after the event. Also my clients that crash diet, stress about college or have weight loss surgery and pregnancy. I always educate them about TE but as you advise see their dermatologist. They do feel better when I let them know its self limiting and will correct over time.

Lei Lei: I went to a dermatologist a couple months ago and she told me my extreme hair fall was due to telogen effluvium, and she said they would grow back eventually, but it’s been 5 years now... and I haven’t seen any regrowth, just more hair loss towards the front of my scalp and even more thinning

Jenifer Jenkins: I just found out recently that I had developed an allergy to the blue dye in my thyroid medication. Every time I would take it I would get a tingling/burning sensation in my scalp, along with excessive hair shed. Switched to a dye free pill and the shedding stopped!! It no longer covers my counters and floor every time I style. Great video! You're a wealth of information!

Rafi I: Dr. Dray, I’ve recently found your channel and have been watching a lot of your videos. I’m a medical student and just finished my second year and am preparing for Step-1. Anyways, I just started Accutane about 2 months ago due to extreme cystic acne and the treatment has been going well. But I have excessively dry scalp and have been losing a LOT of hair. Is there anything you can recommend to help me out here. I’m getting really concerned with my hair loss. Please and thank you.

I have found that I have a much healthier scalp and better skin after cutting out all grains and sugars.....it's been over 7 months since I've been eating this way. The shedding started about 3 months after I stoped and it's still coming out a lot. It's starting to thin slightly around my hair line and I can see more of my scalp... sad, but like you said, it should go back to normal eventually.

Mar Z: Thank you! This was helpful to me. I have thick hair but very few of it. It's a strange combination that most people don't understand because hair loss to them means thinning hair. But i think chronic stress and me tugging at my hair is the culprit. Also, I like that you mentioned that vitamins are not regulated. There is a site called consumer lab and they do independent testing of vitamins to see if the products contain the exact amount and quality of ingredients that the vitamin companies claim. You have to pay for a report but it was worth it to me because it helped me steer clear of certain brands that had higher/lower doses than they wrote on the bottles.

Lauren: I was recently diagnosed with hashimotos a month into my freshman year in college. The past few months, my hair loss has increased significantly. I’m really struggling because my hair has always been my biggest confidence when it comes to appearance, so my self esteem has been really deteriorating lately. Eventually I found something that has completely stopped the problem and that is marine collegan. Got the tip from utuber Schellea Fowler. It also helps my joints as I have osteoarthritis and my skin. Just want to share my experience!

Shera: I was diagnosed with Anegan disease last year, however I noticed my hair stopped growing around 6-5 yrs ago. Now I’m noticing that my hair is falling out much quicker. Im seriously worried I will have bald spots in a few years. I’m guessing all of this is due to all the medications I’m on. I know I can’t stop taking my meds. Is there any advice you would have for me?

Briar Rose: Dr. Dray, are you familiar with Nioxin for hair loss? I've seen a doctor - no vitamin deficiencies, normal thyroid panel - we determined it's hereditary. I'm 68. My hair loss is gradual, progressive thinning all over the scalp, not patchy. Been taking biotin for greater than a year and using minoxidil much longer with some improvement. But now I'm looking at scalp care like skin care. That's why I bought Nioxin. I'd love to know your thoughts. I have longish fine straight hair. I wash it 2X a week and usually let it air dry. By the 3rd day my scalp gets a bit itchy especially in hot weather. If I want a special style I use soft foam rollers or spiral curlers on damp hair. Curling iron on dry hair or salon blowout only on rare occasions. I love your channel and appreciate your advice. What I've learned from you: Ceramides are my friends! Sunscreen every day, indoors and outdoors, even when it's raining! Facial scrubs for feet, not face! I've just started using OTC Differin gel for anti-aging. I'd love to see Nioxin on a future Q&A. It has 3 products: shampoo, conditioner and leave-in scalp treatment.

Destiny Whigham: I heard that biotin only works if you have a deficiency in it already. Eggs, spinach, and cheese are biotin powerhouses if you don’t trust vitamins (I think mushrooms have biotin as well). I was wondering why I never saw results from biotin supplements, then I realized I wasn’t lacking in it, so it wasn’t yielding. Also, vitamin A is the one responsible for hair n nail growth, biotin just helps you absorb vitamins

Rose J: Dr. Dray, I took Accutane twenty years ago for six months and have experienced excessive hair shedding every since.  The volume of my hair is about half what it was.  Luckily, my hair does grow back, to a certain extent at least, or I'd be bald by now since this shedding is constant and shows no sign of slowing down.  Have you heard of Accutane causing this?

Alyssa Bell: Hi Dr Dray. I'm really loving your videos recently, but I have a question about the blow drying and dandruff thing. I have really thick, long hair that takes awhile to dry and am prone to a itchy and flaky scalp. I started blow drying my hair on a cool air setting near my scalp because I was suspecting that the prolonged wetness of my scalp after washing was contributing to the itchiness. Sure enough, once i began blow drying the itchiness stopped and the flakiness decreased. So two questions, one is the cool air setting on my blow dryer also damaging? And two do you have any comment about a wet scalp contributing to itchiness? Thanks so much!

J: Dr Dray, just found your channel. My hair seems to come out in small chunks when rinsing my hair out in the shower. (I imagine it being what would be pulled out if one had their hair pulled in a fight). More comes out when I brush it, like a lot more. I've noticed bald patches towards the sides, above my neck. Also some thinning throughout, especially at the back of my crown. I stress about it daily! What does it sound like to you? I'm too embarrassed to tell my doctor.

Jenny H.: I just finished my 3rd month of Accutane and I started losing hair after being on it for just 3 weeks. It’s still falling out. I asked my nurse and dermatologist if it is permanent and they said that it could be if I have scarring. I lose about 40 strands of hair every time that I brush it and I wash it once a week and when I get out of the shower to brush it I lose around 100 strands of hair. I am very worried that it could be permanent and I don’t know if I should stop the Accutane. My dermatologist isn’t giving me any answers. It’s like she has never seen this before with Accutane. I have bad cystic acne and it really depresses me and I don’t want to stop it but I am so worried about losing too much hair and it becoming permanent. Can Accutane cause scarring and permanent hair loss and is around 100 strands of hair too much to lose in my brush after I wash it? I don’t know what I should do.

Nevena Tanaskovic: My mom's been under a lot of stress ever since I've known her, now on top of everything, my dad has cancer (he used to be a heavy smoker) and her hair is shedding at an alarming rate all of the time.

How much time takes to the body to adapt to the new diet? Someone in my Vegan group recommended prenatal vitamins. Do you think this will help? The only supplement i take is B12. Thank you! Natalie R: It's crazy because I lose A LOT of hair when I brush and in the shower. I do have naturally very thick and curly hair, but it always scares me. I've been like this for many years though and I still have a full head of hair.

SCarol: I'm sorry but, for thinned out curly hair, there's no way it untangles on its own after drying. It just forms huge tangled masses that have to be cut off.

Claudia MM: Hi Dr. Dray! I've been using an Olay spf 15 moisturizer (zinc oxide and octinoxate) and top it off with cotz spf 40. I've been using it like that because I find the Cotz too expensive to apply liberally, but I like the matte finish it provides. Do you think this is a good method? Thanks you!!

adti ashok: I have straight knee length hair doc, but sometimes the ends get brittle if I don't trim for over 6-8 months. I oil and wash my hair twice a week, no heat styling, natural air dry, and wear it only in a loose braid or low bun. I would love your recommendations for a good conditioner to maintain the outer layer of the hair. Thanks in advance and I wish you good hair days!! Here's to no more bad hair days..

Sarah Ashley: I have long term health problems plus I'm a single mum of 3, so both of them combined has led to a lot of hair loss especially in the last few months. I'm sure health and stress is mostly responsible for it and I'm at the end of my tether, there are so many treatments and vitamins ect I have no idea where to start. Otoki Sicily: On supplement regulation: one workaround is to have the doctor write a prescription for a specific supplement, since most pharmacies carry supplements from reputable pharmaceutical brands such as Rugby or Major. If a prescription isn't the best option, another tip is to look for the USP, UL, Consumerlab, or NSF seals on the bottle. Marc Possoff: I’m on a combo of pemetrexed and Keytruda maintenance treatments. My oncologist told me I would experience hair thinning not hair loss. To me thinning and loss is almost the same. I am experiencing my hair thinking and it doesn’t seem as healthy. I do understand these are one of the side effects. I’m a male with shoulder length very wavy hair and I’m not ashamed that I’m vain and take pride. My hair has always been my trademark. With that being said would hair thinning / loss treatments, medication etc be counter productive with my treatments that I get every 3 weeks? Thanks!

Pebbles: I've been to my derm many times over the years begging for help from heredity hair loss. My hair barely covers the top the top of my scalp and sadly this started to happen in my 30's. I don't really feel that science has done anything much to address this issue. I've used Rogaine for years with absolutely no results and different oral supplements, which biotin did help, but not enough that I have anywhere near a full head of hair on top. I keep hearing about new treatments but a couple years ago when I asked my derm about them he told me that it was media hype and the treatment was unavailable. I believe it was something about injecting your own blood plasma (from your donated blood ) into your scalp at the derms office which was completely unfeasable according to my Dr. Anyway my point is, when it comes to hair loss in women if it's severe and makes you cry your best option is to go get a wig or topper so you can face yourself in the mirror, and go on with your life. Toppers are great but you still have to face the people in the gym, atleast I do, because I can't tolerate the added warmth during exercise. If you can find a cure Dr. Dray you'll be Gazillionaire and I'll be the first in line! I think all you derms try to help but honestly for many people, it's hopeless.

aonori71: Thank you so much for your great video!! I am still not sure why my hair falls. Seen by primary provider a year ago, led could be genetic ( my dad is bold). Not using hair drier ( funny because my hair stylist told me to use dryer to prevent hair damage, but I could not keep doing it because I am lazy.) Is it too frequent washing hair everyday? How long did you take to see the effect of hair growth after used biotin ( I started taking biotin last month ) ?

Shelley Nayak: I have never used any heating tools other than being done on salon for a haircut, which occurs only in 6 months or so! Never dyed my hair, no stiff hairstyles, no pulling or scratching on scalp, avoid combing as far as possible and even oils, because oil literally make my dry scalp itchy and trigger hairfall. I am keeping short hair for low maintenance! Still I feel a soreness at root of my hair and hair fall!! Treatment give only temporary results. Can you make a video on shampoo and do they really work as they claim? Ingredients to avoid in shampoo! SLS, Paraben, etc. Thanks.

Godsgirl001: Thanks Dr. Dray. I had major surgery a couple of times and was horrified to see huge clumps of hair coming out. Anyway I am in menopause and last summer my hair became so dry a section broke off. I've since eliminated the beloved flat iron. I love the look of straight hair but would rather have frizzy hair over no hair. Coincidentally I started Bioten Saturday. I'll take it for at least three months and see if I can get some thicker hair. God bless

What would you recommend to someone (me) with extremely greasy hair (but not greasy skin) to the point where I have to use Prell shampoo everyday which helps a bit but goes back to disgusting grease levels in mere hours. Is this probably hormonal? I have dealt with oilier than average hair since middle school and am currently 27. I have been on birth control for endometriosis for 5 years but haven't noticed a difference while on it compared to before. I have yet to come across anyone with as severe hair oiliness. Also, I do not have much dandruff and no hair loss.

polly jetix: I've got a serious problem. Since menopause began, my hair has become extremely fragile and superfine. If I use any hairspray to try to control the flyaways, combing it out tears and breaks the hair shafts. So my dark hair looks speckled, and is very rough. I simply don't know how to manage my hair. If I use no hairspray, I'm like an uncontrolled haystack all over. But the top layer of my hair turns into a disaster area, if I dare spritz it!
